FTI’s traditional impression submission is simple! Start by downloading our Rx form to include with your case.

If you are located within our local pickup area shown below, please schedule a local pickup request. Otherwise, print a UPS shipping label. If you are unsure if you are within our local pickup area, or if you have any other questions please call us at 800-658-1607 and we'll gladly assist you.

If you are located in Kansas, please use the UPS Ground options as your case will deliver to our laboratory next day. For those outside of Kansas please select UPS 2-day to ensure a timely delivery to the laboratory.
